    The information below is here to help you before and after your visit. Whether you want to schedule an appointment, verify that we accept your dental insurance or pre-fill forms, you should find everything you need here. If not, please call us.


    Insurance Plans

    We are an affiliated provider for many insurance plans. If you have another dental insurance provider, we are well equipped to help you manage and maximize your benefits. Whenever possible we will file claims on your behalf for covered services.


    Financial Information

    All co-payments are due at the time of service. Additionally, if a service has been run through your insurance provider in advance to determine the covered amount, you will be responsible for paying deductible and non-covered amounts at the time of service.

    For your convenience, we accept cash, checks, credit cards (Visa, MasterCard, American Express and Discover) and CareCredit.

    We understand that unexpected health care expenses can be a strain on finances. If this happens, please talk with us so we can work with you to determine a manageable payment arrangement.

    Forms & Feedback

    If you are a new patient, you can complete all of the new patient paperwork prior to your visit. You can fill out the form online or print it and bring the completed form to your appointment.


    Scheduling Appointments

    As an established patient, you can login to Patient Account Access to review your account online, schedule appointments, make payments and much more.

    You can also schedule or change appointments by calling us at 918-542-4442 between 8:30 a.m. and 5 p.m., Monday – Friday.
